It still flashes up in the taskbar. On Windows 7, if you already have
a command prompt open, its taskbar icon sort of jiggles about and
briefly glows up blue. Still don't like it.

What variables do actually need to be set before the first Cygwin
process, i.e. couldn't be set on the mintty command line or in shell
startup scripts?

In any case, I suspect only a small minority of users do make use of
that, and they do have the option of creating a batch file themselves.
(The mintty manpage actually mentions that possibility and the need to
invoke it through 'start'.)
