Several people have now clued me in that the HTML Help XSL stylesheets
were created by Jirka Kosek (to sit on top of Norm's HTML stylesheets) and
indeed I found his name in the ChangeLog and all over the stylesheets.
So my apologies for not giving credit where it was due. Thank you,
Jirka, for creating these stylesheets!
