You want to use Norm Walsh's xsl stylesheets
<http://sourceforge.net/projects/docbook/> to transform docbook source,
right?

Some of the items you mention are parameters and others are controlled
with the template system. See:
http://www.nwalsh.com/docs/articles/dbdesign/#selfcust 

If something is not already parameterized, you can override particular
templates to get the results you need or submit a feature request at
sourceforge. 

And when you get stuck, you can search the archives of this list or post
a question here :)
