PassiveTeX table borders

Subject: DOCBOOK-APPS: PassiveTeX table borders

With Jeff Beal's help, I've produced a patch to the fo/table.xsl file that
will put the missing left-hand border on tables produced using PassiveTeX.
This is much simpler than the convoluted, dual-table kluge I posted a
couple days ago:

Of course, this will probably have to be overridden to produce borderless
tables :-/
