I recently implemented and described some workarounds for getting
properly collated page numbers out of FO. The same technique might work
for RTF...
I'm offline so I can't send a pointer. Basically you output markup
that contains the page numbers. So in pass 1, if you printed your
index it would look like it contained index markup. You extract that,
massage it, and reformat the document using that index.
