Actually, the 'use.extensions' parameter enables the
XSLT extension functions, and has nothing to do with
the 'passivetex.extensions'. Of course, that
isn't obvious from the parameter names.

That's odd. I've never seen PassiveTeX do that.
Can you peek in the FO file and see if the pages
are repeated there?

The passivetex.extensions will turn on the PDF bookmarks feature.
Yes, there are still formatting problems with PassiveTeX.
