[docbook-apps] footnoteref problems in PDF


I have a fairly large table (five columns, twenty-six rows) and every entry
in the table contains a <footnoteref> referencing one of two footnotes. I'm
using DocBook SGML 4.1, the current DocBook DSSSL stylesheets, and OpenJade
1.3.1. My HTML output looks pretty much as I expected: after the table, I
have two footnotes.

The PDF, however, includes an instance of each footnote corresponding to
every <footnoteref> in the table: 130 footnotes spanning six pages.

Any idea how I can fix this?

Is this a stylesheet bug or an OpenJade problem?
