I don't know of a way to do that in fo, but that's not saying much. I'd suggest asking on one of the fo lists or on the fop list--people there have many tricks up their sleeves.
If you do find a way to do it with fo, I'd be interested to hear how.

The document itself needs to have margins around it. The idea is to use
a page-sized image as a background, overlaid by the actual document.
