<?dbhh ...?> is supported by stylesheets out-of-the box. As aliases are 
useful only in HTML Help they are captured in HTML Help specific 
processing instruction.

It is a fragment identifier. It is autogenerated in a case that you 
don't have id attribute on corresponding element (typically chapter or 
section).
