Regarding the renderas attribute in bridgeheads, you should take a look
at the match="bridgehead" template in fo/sections.xsl.  You'll see
that a bridgehead is handled with the 'section.heading' template,
with a heading level chosen based on its container element.  In general,
it computes the container section level and adds 1.  If the bridgehead
is not in a section element, then the bridgehead level is considered to be
2.
I think that is to prevent it from being confused with an actual
level 1 heading, I suppose.
