Michael Smith is, of course,  correct in observing that named character
references can be handled on another level. Given a standardized mapping
between character names and character set code points, either the tool alone
or some standardized character set declaration supported by the tool will
make it more or less transparent for the user. The point is that there are
standardised mappings between character names and character set code points;
i.e., it makes sense to actually store character code point values rather
than character entity names.
