The htmlhelp stylesheet adds some OBJECT elements. Check out
htmlhelp/htmlhelp-common.xsl in the DocBook XSL distribution. You probably
won't have to use the awkward disable-output-escaping mechanism, because you
aren't using the Microsoft Help compiler. The example in the template named
'write.indexterm' doesn't use that mechanism, and is probably closer to what
you are looking for.
You can also see this reference for some methods for inserting code in HTML

There is an example using a processing instruction to trigger code
insertion.
