Well, if FOP won't do it correctly, then fake it!  Adding a matching color
border top and bottom seems to work in FOP:

  <xsl:attribute name="background-color">#A09A98</xsl:attribute>
  <xsl:attribute name="padding-left">5pt</xsl:attribute>

  <xsl:attribute name="start-indent"><xsl:value-of
select="$title.margin.left"/> + 35pt</xsl:attribute>
</xsl:attribute-set>
