DOCBOOK: Hyperlinks in Adobe Acrobat

We are authoring in XML using Adept Publisher. We use a customized version
of the DocBook XML DTD, and we are using a FOSI to format our printed
output. 

I would like to publish Adobe Acrobat files with hyperlinks in the Table of
Contents, Alphabetical Index and the internal cross-references. Is there a
way to automate the creation of these links in the Acrobat files?
