I have. I have done the following:
* used db2pdf (with RH 6.2) which runs it at least twice
* use pdfjadetex by hand and ran it FIVE times
I still get no index references. Interestingly, when one of my
<indexterm>'ed words appears NEAR a figure with an id, and an xref
pointing to it, the page numbers are inserted correctly. Nonetheless,
I've tried small one page documents to index, and no matter whether I
give the indexterm an id or not.
This, for example, doesn't work:
