It can't be done with just stylesheet parameters
in the stock DocBook stylesheets.
It requires a stylesheet customization file
containing a custom XSL template. See:

You should look at the Website extension to DocBook.
It lets you set static links and images in headers and
footers with parameters in a configuration file.
But it assumes you are writing individual web pages,
not books. See:
