[docbook] Profiling and Indexes

I'm experiencing some strange behavior moving to profile-chunk.xsl (from
chunk.xsl).  The index is now empty, the html page for the index is created
and participated in the toc but is empty with exception for the navigation
and title. None the indexterm content makes it. (The indexterm blocks are
not involved in 'userlevel' elements used to hide specific paragraphs and
notes)

Any suggestions on possible cause?
