[COMMIT] Get rid of tm-ia64.h

- Subject: [COMMIT] Get rid of tm-ia64.h
Some more low hanging fruit. Checked that ia64-linux and ia64-elf
targets still build. Committed as obvious.
Thanks to Andreas & Kevin for helping cleaning things up.

Don't include "tm-ia64.h".
* config/ia64/ia64.mt (DEPRECATED_TM_FILE): Remove.
* config/ia64/tm-ia64.h: Remove. Move contents ...
* ia64-tdep.h: ... here. Update copyright year.
