[4/8] remove call-cmds.h


This removes call-cmds.h.

This file exists pretty much to provide a single declaration, of a
function that has a single caller in the tree.  Also, the file is marked
as deprecated in a comment at the top.

This patch removes the file and explicitly declares initialize_all_files
in the appropriate spot.

	* Makefile.in (init.c): Declare initialize_all_files;
	don't include call-cmds.h.
	* call-cmds.h: Remove.
	* symtab.c: Don't include call-cmds.h.
	* top.c: Don't include call-cmds.h.  Declare
	initialize_all_files.
