[6/10] RFC: split exception catchpoints into a new file


This is a relatively simple restructuring to make the code cleaner,
especially in light of some subsequent patches.

This moves the C++ exception-related catchpoint code into a new file,
break-catch-throw.c.

This has a couple minor caveats:

* It exposes ep_parse_optional_if_clause.
  This is just a utility function, so no big deal.
  Perhaps it ought to be renamed.  I don't know what "ep" means...

* This removes a call to breakpoint_adjustment_warning from the code.
  I think this is reasonable -- users shouldn't really care about the
  precise breakpoint location chosen by gdb for these catchpoints.

Built and regtested on x86-64 Fedora 16.
