Yes, you are correct. You cannot use b20 to build Insight. Of course, this bug has sat around so long that you probably already have something working.
On windows/cygwin, Insight does not use X, as you suspected. As I recall, the imagehlp.h propblem was resolved, but I don't remember anymore by what. You should be able to build 5.1 without incident.
