xmlinst.exe is placed in your system folder along with mxsml3.dll.
IE5 XSL has no built-in templates, so you need to be careful about the root
pattern. Try this:
<xsl:template match="/">
<html>
<xsl:apply-templates select="a/b"/>
</html>
</xsl:template>
