My own problems with finding the relevant material in the specs (and the
XSLT a XPath are fortunately better organized than others) gave me the
strength to spend quite a few hours with the direct linkage to the
specifications in:
http://www.zvon.org/xxl/XSLTreference/Output/index.html ([go to
standard] in the menu bar)
