It is not the XSLT processor that is having trouble; it is the XML parser that the XSLT
processor is relying upon. A general purpose XML parser isn't a MathML processor. When
using an XML parser to read MathML, you will have to declare the entities that you
reference. The parser is trying to replace the references with their replacement text.
The XSLT processor never sees the original entity reference, so you can't copy it through
to the output.
