I have upgraded cygwin.

$ uname -sroi
CYGWIN_NT-5.0 1.5.5(0.94/3/2) unknown Cygwin
$ cygcheck -c | grep cygwin
cygwin 1.5.5-1 OK
$ g++ --version
g++ (GCC) 3.3.1 (cygming special)

Now pthreads and cout work fine.
